The family doctor writes down your symptoms in a doctor's referral letter.  This letter is for the doctor that will be helping you further with your symptoms, for example, a doctor in a hospital or an optician (eye doctor) if you need glasses. You take the letter of referral along with you to your next appointment with this other doctor.
